Completion of this report is not an admission of liability by the facility or customer. By using our wash, customer has assumed responsibility for loose, defective, previously damaged, or non-factory original items as well as un-lowered antennas. Of course we will pay for damage caused by a mistake on our part, but we require a full investigation prior to any actions.


Full and complete responses to all of the requested information will assist in the investigation and processing of your potential claim. Failure to provide all of the requested information may delay or adversely affect the investigation and processing of your potential claim. Thank you for your time and cooperation!

Please attach pictures and supporting documents. If this is a vehicle claim, a picture of the VIN must be the first attachment and the tag for the second picture. Five images are required to process any claim.

I also understand that submitting this form does not indicate that The Wash Method has accepted responsibility for this matter and that responsibility will be determined after further investigation and analysis of the facts

    and circumstances relating to the incident.
